The Circle of Care Coordinator is responsible for leading a collaborative, community-driven approach to supporting children and families through Jordan’s Principle. This role focuses on coordinating Healing Circles, developing wellness plans, and ensuring wrap around services are delivered in a culturally grounded and holistic manner.

Working closely with families, Brokenhead Ojibway Nation (BON) departments, and community partners, the Circle of Care Coordinator ensures that all services align with the principles outlined in the Jordan’s Principle Operational Policy and Procedures Manual and the Wii Bimaadizewag Abinoojiiag Act (Our Children Will Live Act), while reflecting the best interests of children and families.

Key Responsibilities:

  • Establish and maintain the Circle of Care Committee
  • Build and maintain collaborative relationships with BON departments, the Lodge, and other community partners
  • Coordinate Healing Circles with children, families, and identified supports to develop wellness plans and wraparound services
  • Monitor wellness plans and track progress of children and families
  • Collect program data, prepare reports, maintain accurate documentation of all meetings and activities, and attend all training required to perform duties
  • Coordinate meeting logistics, including scheduling and room bookings
  • Maintain Circle of Care agreements, including Memorandums of Understanding and Confidentiality Agreements
  • Ensure all work aligns with the Wii Bimaadizewag Abinoojiiag Act (Our Children Will Live Act) and the Jordan’s Principle Operational Policy and Procedures Manual
  • Deliver services in a culturally grounded, family-centred, and prevention-focused approach
  • Support and empower families throughout their healing journey

QUALIFICATIONS AND SKILLS REQUIRED:

  • Post-secondary education in Social Work, Child & Youth Care, Human Services, or a related field (or equivalent experience)
  • Experience working with children, youth, and families, preferably in a First Nations or community-based setting
  • Strong understanding of holistic, culturally grounded approaches to care
  • Knowledge of Jordan’s Principle, including operational policies and procedures, is a strong asset
  • Experience facilitating meetings and coordinating services
  • Strong organizational, communication, interpersonal skills, and conflict resolution
  • Ability to manage confidential information with professionalism
  • Clear Criminal Record Check and Child Abuse Registry Check (required)
  • Willingness to submit to a pre-employment drug test and future random drug testing
  • Ability to provide three (3) references upon request
